High School Sports Physicals to be Offered at St. Joseph Medical Center

Providence St. Joseph Medical Center is offering high school sports physicals on select dates in June, July and August. The cost of the physical is $30 (no insurance or credit cards accepted); all proceeds are donated to the Polson Booster Club to support athletic programs.

Prior to your child's exam, please visit the Montana High School Association website to download and print a Pre-participation Physical Exam Form.

2018 sports physical exam dates & times

• June 12 | 3:30-6 p.m.
• June 28 | 3:30-6 p.m.
• July 17 | 3:30-6 p.m.
• August 7 | 3:30-6 p.m.
• August 9 | 3:30-6 p.m.
