PROVIDENCE OREGON – Flu season is coming soon, but there is a simple step you can take to protect yourself, your family and your patients: Vaccination helps keep you healthy and reduces your chance of passing illnesses to others.

Clinicians must get a flu shot or decline by Oct. 31.

On-site vaccination clinics

Walk-in vaccinations clinics at hospitals are planned at the dates and times shown at the end of this update. You also can make an appointment with caregiver health by calling 503-216-3200.

Reporting vaccination status

You must report vaccination status to caregiver health if you:

• Receive your vaccination elsewhere, such as a primary care clinic or pharmacy. Download attestation form.
• Cannot get a flu shot. Download declination form.

If you’re employed by Providence can report your vaccination status online, on the caregiver health intranet site.
