Schedule your flu shot! Vaccines protect you and those in your care

Health care workers are at high risk of getting and spreading influenza to their patients, colleagues and family members.  

Why it matters: The flu can cause serious complications and even death, especially for people with chronic conditions or weakened immune systems. 

Preventing the flu is up to you: Annual flu shots are an important way to protect others from a potentially deadly disease.  

What to do

All caregivers are asked to get a flu shot or decline by Oct. 31.

Providence-employed clinicians:

• Find a caregiver vaccination event near you. Caregivers can get a free flu shot through caregiver health services. 

• If you get a flu shot through your provider or pharmacy, complete an attestation to inform the caregiver health team.

• If you cannot get a flu shot, submit a declination form.

Other medical staff (includes links outside the firewall):

• Find a caregiver vaccination event near you, to get a free flu shot through Providence caregiver health services. 

• If you get the shot elsewhere, print and complete the attestation form. Then return it to any caregiver health services location, email Oregon.RegionCaregiverHealthServices@Providence.org, or fax 503-215-6831.

• If you cannot get a flu shot, print and complete the declination form. Then return it to any caregiver health services location, email Oregon.RegionCaregiverHealthServices@Providence.org, or fax 503-215-6831.
