Submit your required COVID vaccination by Oct. 18

Caregivers and providers who are unvaccinated must act now to continue working after Oct. 18, 2021. Caregivers and providers are required to get vaccinated and submit their vaccination record, or request a medical or religious exception.
Please note: If you are an affiliated provider, you must work with your employer to ensure you are compliant with COVID vaccine requirements. You must be compliant to enter a Providence facility.

After Oct. 18, by Oregon mandate: You will be unable to work in a Providence facility if you are not fully vaccinated and haven’t requested a medical or religious exception.
How to get vaccinated or submit a medical/religious exception
Find a vaccination site

Submit a vaccination record
Submit your vaccination record to Providence's COVID vaccination registry. Verify that your information is up to date on the vaccination registry.
Request a medical or religious exception
Exceptions must be submitted by Oct. 18, 2021. Complete Providence’s online exception request form.
Medical exceptions (caregivers and providers)
In addition to the online form above, your provider will need to complete and sign a form that is available in the online exception request tool.
The recognized medical contraindications to the COVID vaccination can be found on the Centers for Disease Control and Prevention (CDC) website.
You also can review with your provider the recognized medical contraindications and any medical condition you may have.

Religious exceptions
A religious exception request is available for sincerely held religious beliefs. Personal preference or objections to mandates are not considered grounds for a religious exemption.
