Deadline extended, action needed on flu shot status

Oregon caregivers and providers who have not declared their flu shot status now have until 11:59 p.m., Monday, Nov. 14, to do so.

If you didn’t get your flu shot from Providence Caregiver Health you must report your vaccination status using the instructions below. 

Action needed 

Report your vaccination status on the caregiver health intranet site by Nov. 14

  • If you received your vaccination elsewhere (for example, at a provider’s office or pharmacy).
  • If you’re not getting the vaccine this year. 

 

Reporting for Providence-employed providers

You can scan the QR code below to get to the caregiver health site. Then scroll down to “Tell Us!” for step-by-step instructions (screenshot below).

Reporting for other medical staff members

If you are a provider who is not employed by Providence, email a completed flu attestation form or send a copy of the documentation that you received when you obtained your flu vaccination. Please include your name and DOB to: Caregiver Health Services at oregon.regioncaregiverhealthservices@providence.org

If you’re unable to receive the vaccination for religious reasons or medical contraindication, you must complete an influenza declination form and send to Caregiver Health Services at oregon.regioncaregiverhealthservices@providence.org. Masks already are required due to COVID protocols.
